New Jersey Governor's Council on End-of-Life Care

Greetings and Welcome to the 2017 End-of-Life Care Survey! 

You have been asked to participate in this survey, which is being conducted by the New Jersey Governor's Advisory Council on End-of-Life Care with support from the New Jersey Department of Health.  The purpose of this survey is to gather information from New Jersey residents who may be impacted by decisions and conversations on End-of-Life Care.

Your responses are strictly confidential and will be anonymously collected, as personal identifiers are not obtained nor connected with the data you submit.

Instructions for Completing the Survey

This anonymous survey should take approximately five to ten minutes to complete. It is intended to be completed in one sitting by you only. Please do not delegate this to anyone else.
