Submission Instructions

Thank you for your interest in the New Jersey Influenza Honor Roll!

The New Jersey Department of Health (NJDOH) is continuing the annual New Jersey Influenza Honor Roll (NJ IHR) during the 2023-24 flu season. The NJ IHR recognizes institutions that are striving to promote influenza prevention at their facilities.

Increasing vaccination coverage and continuing to protect the health of New Jersey's citizens requires efforts from multiple stakeholders within our communities. These stakeholders share in the responsibility to helping ensure that our communities are aware of the dangers of flu, increasing awareness of the benefits of vaccination, and improving access to vaccination services. Institutions submitting applications to be considered for the Influenza Honor Roll must be committed to taking an active role in improving the health of their students, employees, patients, and/or communities.


ELIGIBILITY Four categories are eligible to submit applications for The New Jersey Influenza Honor Roll: Businesses Community-Based Partners Institutions of Education Healthcare Facilities Activities for consideration may include (but are not limited to): Flu prevention campaigns Flu vaccination clinics; and/or Partnerships with local stakeholders to promote flu awareness and prevention
SUBMISSION PROCESS Submissions for the NJ IHR will be accepted through March 31, 2024. Submissions will be accepted through one of three methods: Completing this survey. (Note: This survey must be completed in its entirety. It cannot be saved and completed later. You may complete the survey and then submit supporting documents to IHR@doh.nj.gov. Faxing your submission to 609-826-4866, attention: Influenza Honor Roll, or Emailing your submission to IHR@doh.nj.gov. If you register and do not have supporting documentation at this time, please submit supporting documents separately to IHR@doh.nj.gov Someone from the NJ IHR may reach out following your submission if any additional information is needed.
HEALTHCARE FACILITIES Healthcare facilities may also apply to the Immunization Action Coalition’s (IAC) Influenza Vaccination Honor Roll for state and national recognition. Facilities that are added to IAC’s Honor Roll will be cross-promoted on the New Jersey Influenza Honor Roll. To be added to the NJ IHR, please forward a copy of the submitted IAC application, if you registered with IAC first. If you wish to only register for the New Jersey Influenza Honor Roll, click next. Healthcare facilities that have been previously added to the New Jersey Influenza Honor Roll do not need to re-apply annually.
AWARDEE NOTIFICATION Awardees will be notified by May 2024. Those who are added to the NJ IHR will receive a certificate of recognition, will be added to the NJ IHR webpage, and will receive a web badge to further promote their achievement.
